SCS/HCS/HB 1713 - This act permits birth parents to obtain, upon a showing of proof of identification, a copy of an adopted person's original birth certificate. The adopted person may request from the state registrar a contact preference form to accompany the original birth certificate. Additionally, a deceased adopted person's lineal descendants shall have the right to obtain a copy of the adopted person's original birth certificate and accompanying contact preference and medical history forms in accordance with existing statutory provisions relating to birth parent contact preferences and medical histories.
