HCS/HB 1713 - This act permits the birth parents to obtain, upon a showing of proof of identification, a copy of an adopted person's original birth certificate. The adopted person may request from the state registrar a contact preference form to accompany the original birth certificate. Additionally, a deceased adopted person's lineal descendants shall have the right to obtain a copy of the adopted person's original birth certification and accompanying contact preference form and medical history form.
