HB 1265 - Under this act, declarations of candidacy for election to office in any city, town, village, county, or special district, or for the General Assembly or any statewide office must contain a candidate's last name or maiden name as it appears on his or her birth certificate or his or her legal last name as changed through marriage or court order.
