SB 122 - This act requires health insurance carriers which
offer insurance policies to use a standardized form for insurance
identification cards issued to the insured. Specific information
is required on the standardized identification card, and the
Department of Insurance shall develop it in consultation with
insurers and physicians. These requirements go into effect upon
January 1, 2000.
