SCS/SBs 569 & 578 - This act contains two requirements for information placed on documents issued by health insurance carriers and third party administrators.

Health insurers and administrators must provide specific information on the explanation of benefits form given to the health care provider whenever a claim is paid or denied. Health insurers and administrators must use a standardized form for insurance identification cards issued to the insured. Specific information is required on the ID card, and the Department of Insurance shall develop it in consultation with insurers and physicians.

These requirements go into effect upon January 1, 2000.
